Change of Name or Address
Name and/or address changes must be recorded with the Admissions & Enrollment Center. Addresses are verified by U.S. Mail for resident tuition purposes.
When changing your name, supporting documentation must accompany the Personal Data Change form. Supporting documentation may include a copy of a marriage certificate or court order. Students who change their legal residence must complete a Personal Data and Academic Program Change Form in the Admissions and Enrollment Center or download the form online. Students will be asked to provide proof of resident status.
